Form for bankrupts who are operating a business in their own name. This form must be completed monthly together with the Monthly Income & Expense Statement.

Tax Form
Please complete the information on the enclosed forms and return to us, attaching all tax slips (T4's, etc) and related tax information. Please keep a copy of your slips, especially if your spouse is not bankrupt, as you will need to know your income when filing their return.
